If the cat had fleas he'd be constantly scratching and nibbling himself. Not licking. My cat had fleas twice so I know (VOMIT. GOD I HATE FLEAS). Also you can SEE fleas. You'd know.

Cats twitch their fur. It's more noticeable along the backbone because there isn't much meat or fat in that area. Sounds normal to me.

But if it's as extreme as you say, it's possible he has some neurological issue or something. It doesn't sound like a skin issue.

My cousin had a cat with similar symptoms (he did other crazy stuff too) and it was neurological. If it makes you feel any better, the cat probably isn't in pain, just confused.

Maybe time to see a vet and get a diagnosis.
