Birds usually sleep on one foot. Losing a toe wouldn't cause problems unless the bird had some other issue that made balancing difficult.
I don't think it's physically possible to survive for 18 months without sleep. The bird has to be sleeping and his owners are too dumb to figure it out. It's pretty obvious that they don't know much about bird care.

well, if they made the cage for disabled friendly, he would be able to sleep better. he also has a broken toe on the other foot...

It said he has not had "a good night's sleep" for 18 months. I'm glad they're finally doing something to help him.
