Need help filling out DNA form.....

Ok I just recieved in the mail the DNA form from Animal Genetics. I am doing the Blood Sample and I need help filling the form out. Yes I can read but I have never done this before and I don't want to do it wrong. The form has two sections and I don't know what to fill and what not to fill. I read somebody else did this can you please help me fill out the form? Oh by the way do you need the bird's ID? My bird was not banned so Nimbus does not have an ID number. What do I put or do I leave it blanck? Help would be appreciate it.

You need to fill in both sections. The top sections is your name etc. Put the species of you bird and at the band section just put his name. Mark the sample block for vial or card- sex testing if that is what you are doing. Also mark the block for dna sexing a little lower down. Complete the payment particulars.

The test kits I use are a tiny bit of paper inside a vial. Be carefull not to touch the paper inside the vial when you collect the blood. You need to hold the outside edge only if this is the type of kit. The blood only needs to be visable on the inside bit.

Thank You very much 2horse! I am doing the DNA sex id. with the card.I just have to put a little blood on the circle indicated by the card and then send in the card. I hope I can get the money this week and send the card in by monday. Thanx again.
